Program Overview

We are looking for a registered nurse to join our nursing staff in Canada providing excellent health care to our patients. Your duty of caring for the patients coming into our facility include tasks like creating and maintaining accurate patient records, administering medication and monitoring for side effects, recording vital signs, and reporting symptoms and changes in patient conditions.

A bachelor’s degree in nursing and a state license are essential requirements for consideration. You will also need to be trained in BLS/CPR and be ACLS-certified. A professional, friendly attitude and effective communication skills are keys to becoming successful in this role.

The Lower Outaouais Family Health Team (LOFHT) is looking for a dedicated permanent full-time Primary Health Care Nurse Practitioner (NP-PHC). The LOFHT serves our community by promoting wellness and healing through collaborative care, where the team works together to support active participation of the patient towards their wellness goals. Located in Hawkesbury (half way between Ottawa and Montreal) our 24 family physicians and interdisciplinary health professionals work collaboratively to provide the best primary care to our 19,000 patients.

Main Responsibilities/ Position Focus:

2 Positions are available and can consist of a mix of the following:

Collaborates and/or consults with physicians and other interdisciplinary team members to provide high quality primary care

Chronic Illness Management

Woman’s health clinic, cancer screening

Well baby program

Same day appointment and after-hour clinic (if needed) for acute care issues

Education, Experience and Skills:

Current extended class certificate (NP-PHC) and unrestricted registration with the College of Nurses of Ontario

Current membership with RNAO or NPAO, including professional liability insurance coverage

At least two (2) years clinical experience will be considered an asset

Certification in CPR (BLS-C)

Ability to work effectively both independently and as part of a team

Mental Health experience would be an asset

RESPONSIBILITIES

• Ensure resident care is provided with established nursing standards and principles, physicians’ orders and administrative policies, as well as, observe, report and record vital signs, symptoms and conditions of residents. Assure observance of required techniques in nursing care.
• Assist in assessing residents and develop, implement, review and evaluate resident care plans. Monitor residents’ health, notes changes in condition and assess need for referral to doctors; processes and implement physicians’ orders.
• Plan, direct, supervise and evaluate the work of nursing and personal care staff assigned to the unit. Orient and train new personnel and may participate in in-service education. Monitor performance and conduct performance appraisal and refers problems to the Director of Care.
• Assist in preparing the Facility for RAI MDS.
• Take charge of the Facility in the absence of the Director of Care.
• Staff replacement in the absence of the care clerk.

QUALIFICATIONS

• Appropriate current certificate of competence from the College of Nurses of Ontario required.
• Registration or eligible for registration with the Registered Nurses Association of Ontario.
• Previous managerial experience in long-term care preferred.
• Experience dealing with the cognitively impaired.
• Comprehensive knowledge of nursing and health care practices, as well as Knowledge in Infection Control Practices.
• Demonstrated commitment to working with seniors.
• Ability to work effectively individually and with others while prioritizing tasks to handle the most urgent first.
• Ability to present information clearly and effectively both verbally whether in person or over the phone, and in writing; an active listener who projects a friendly demeanor.
